How to use this guide
Our kit consists of the following:
- A swab.
- A transport medium in a tube (this also has the kit barcode or reference number pasted on it). After taking the swab, place it inside this tube.
- A lab instruction form with guidance on sample collection.
- A security seal.
- A transparent plastic bag that will hold the tube with the swab to prevent any leakage in case of breakage.
- A postage-paid return label. This will be pasted on the box.
- A cardboard box to carry the swab, tube, and plastic bag back to the lab.
1. Perform your self-swab sample
- Take care to read the enclosed laboratory instructions carefully so you can correctly take the swab sample.
- This is a very important step as not following instructions can invalidate your results, you will still be charged for the test.
- Please note down the kit barcode number (this is found on the sample tube containing transport medium included in your kit) BEFORE closing the box.
- Please make sure that the box is securely closed, and the flaps are tucked inside the box. Then attach the security seal to the flap and bottom of the box, in the areas marked.
2. Activate swab and complete questionnaire – IMPORTANT
- Activate your swab by completing an online form (you will need the kit barcode number). Do this as soon as you have performed the swab.
- Please enter the barcode number carefully because if it is incorrect, it will invalidate your sample. PLEASE TAKE CARE TO ENTER THIS CORRECTLY AND MAKE SURE THAT YOU DO NOT RE-USE THE BARCODE NUMBER IF YOU HAVE MORE THAN ONE SAMPLE KIT.
3. Take a note or picture of the tracking number on the pre-paid return tracking postage
- Your kit comes along with a Royal Mail Tracked 24 Return Label. Please note down the tracking number as you will need this later to track your swab.
4. Post back your sample to our lab
- Due to the pandemic, you MUST post your swab sample in a Royal Mail Priority Post Box normally before 5.30 PM Mon-Fri. If you are posting on a Saturday, you must normally post before 11:30 AM to make the last post collection. Please re-check the last collection time based on your specific location.
- You can find your nearest Priority Post Box at https://www.royalmail.com/services-near-you#/ (enter your postcode and look for Priority Post Box on the right-hand side pane).
- We strongly advise that you organize your own courier to guarantee the sample reaches the lab on time as there can be delays in the postal system. The Flame Lily will not be responsible for any delays or the sample not reaching the lab in time.
5. Check if the swab has been delivered
- Check the tracking number to ensure that the lab has received your sample on the next working day.
6. Receive results
- You will receive your results as a PDF attachment via email.
Please complete the following form to receive your test result certificate.
Please fill out a different questionnaire for each individual whom you require a test certificate/result. Failure to complete this form correctly may invalidate your test result.
If you test positive for Covid-19, your contact details will be passed on to NHS Test & Trace.

* Please note it is your responsibility to ensure that the swab is taken within the time window required for your flight (normally 72 hrs or 96 hours). The time entered here will be the time that is present on your fit-to-fly certificate as the time the swab sample was taken.
